  Jay brings over a decade of experience from his tenure as a Deloitte Consultant, where he mastered the art of driving growth in the healthcare sector. As the former CIO of a major Blue Cross Medicare unit, Jay played a pivotal role in establishing new health insurance companies. He now leads our Chronic Care Connect Foundation as the principal growth executive and visionary, dedicated to enhancing the health and well-being of patients with chronic conditions.  

Jay lives in and works from Wisconsin

 Yulianna, with extensive experience in building high-performing clinical teams, ensures our nursing staff delivers top-notch care. At the Chronic Care Connect Foundation, she enhances CCM programs to meet our mission of improving health for patients with chronic conditions. Her leadership solidifies our role as a thought leader in chronic care management. 

    Yulianna lives in New Jersey and works in New Jersey and New York 

 Bob served as the CEO of a  successful Venture-backed technology firm, after being recruited from his Partner-Level role at Deloitte.  

Between leading world-wide consulting at a major healthcare software and outsourcing firm, and growing other people's businesses, Bob experience and credentials strengthen ability to grow.
