Every dollar you contribute to the Chronic Care Connect Foundation releases 7 times more Medicare dollars per year to your own physicians and their patient care. And moves your physicians into a rare group of providers who can state exactly why they do a better job of patient care on CCM.

Your donations are dedicated to patient outreach, education and enrollment into Medicare's CCM Program. Once enrolled, Medicare pays for monthly telehealth connections by certified mid-level providers, giving your Chronic Care patients ongoing clinical attention.

We know your physicians are already as busy as they can reasonably be. So when you pay to get a patient enrolled in the Medicare CCM program, you give those providers a helping hand ... and generate new income for every TIN without having to ask more of the physicians.
