Physician-Driven Programs

The Problem

Medicare offers monthly support for patients with Chronic Conditions, under a specific program they call Chronic Care Management (CCM). Unfortunately, physicians and patients are generally unaware of the program and how to access it. Equally important, there is a big funding gap between awareness and program access. We close that funding gap.

Patients who access this wonderful Medicare CCM program receive regular monthly telehealth-based nursing consultations, ensuring comprehensive, coordinated care to enhance quality of life. For each enrolled patient enrolled, physicians receive a monthly Medicare reimbursement above and beyond their other CCM activities.

But the process of education and enrollement is not covered by Medicare. As a result, less than 3% of all physicians currently participate. Most don't even know about Medicare's CCM.

Our Mission

Our mission is to educate chronic care patients and their physicians about the Medicare CCM program. With awareness, we provide staff to help physicians establish a program, and help enroll patients. But we need your help. A contribution of $100 releases $720 or more per year in support for one Medicare patient suffering from a Chronic Health condition.

The Medicare CCM program is not the only virtual resource for Chronic Patients. But not all virtual resources have equal value.

Our clinical staff continuously reviews websites for content that will help quality of life for people with COPD, Alzheimers, Diabetes or any of the other chronic conditions specifically identified by Medicare.

The sites we recommend have been reviewed by this staff to assure that the advice, support and information is reliable.

Curated content is freely available on this website. Patients who want a little more help can chat or phone our nursing staff to help find and select content most relevant to them, at no charge.
